Sports Physical Information:
The 2024-2025 Sports Physical Packet must be completed if you would like your child to have their physical completed in the School Based Clinics.
If you prefer to take your child to their Primary Care Provider, the TSSAA Sports Physical Forms will need to be completed and returned to your child's School Nurse.
All Monroe County High School students are required to complete and return the Urine Drug Screen Consent prior to sports participation to their School Nurse.
