Monday, October 27 10:00am – 4:00pm
Tuesday, October 28 10:00am – 6:00pm
Wednesday, October 29 10:00am – 4:00pm
Thursday, October 30 10:00am – 4:00pm
You MUST Bring:
1. Birth Certificate of each child registering or SS Card
2. Picture ID of parent or legal guardian,
3. Legal Guardianship Papers (if not the parent)
4. Proof of Income: Check stub, food stamp benefits
letter, etc.
5. If you receive food stamps you HAVE to bring the
Food Stamps Benefit letter. You can get a copy of
that letter at the DHS office.
6. Proof of Residency: Utility Bill
