Athletes… it is almost time for your sports physicals! You will need to know if you are getting your physical completed at the school clinic or at your doctor. Once you have that information please come by the clinic to pick up your packet.

Return school clinic packets once they are filled out so we can get you scheduled with the nurse practitioner.

Pick up packets in clinic (for both school clinic and doctors office)  and return ASAP!
